ABSTRACT

Introduction. Children with autism spectrum disorder (ASD) have difficulties adapting to stressful situations, such as the COVID-19 pandemic. Objective. To assess the impact of the first year of the pandemic on feeding difficulties in children with ASD. Population and methods. The caregivers of children and adolescents with non-syndromic ASD (age: 218 years) from a neurodevelopment unit were invited to participate. Participants completed a questionnaire and the Behavioral Pediatrics Feeding Assessment Scale (BPFAS) online before and during the first year of the pandemic. A priority score (product between the average frequency and the problem) for each BPFAS item was estimated. Results. Among the caregivers, 56.6% (86/152) completed the survey (mother: 74.4%); children's median age was 6.3 years (p25p75: 4.78.2); 80.2% of children were males; 58.1% continued with their therapy during the pandemic; and 61.6% had behavioral problems before the pandemic. During the first year of the pandemic, children had greater behavioral problems (34.9%) and feeding difficulties (61.6%); however, 31.4% of caregivers referred improvements in feeding. There were no significant differences in the BPFAS scores before and during the pandemic. The following items obtained the highest priority scores: gets up from table during meal, does not try new food, does not eat vegetables, does not eat fruits. Conclusions. A high frequency of feeding difficulties was noted during the first year of the pandemic. No differences were observed in feeding characteristics before and during the pandemic.

ABSTRACT

Abstract Avoidant or Restrictive Food Intake Disorder (ARFID) is an eating disorder (ED) not common in adults. In this article we present a clinical case of ARFID in a 37-year-old male patient treated in an ED center in Medellin, Colombia; displaying anxious symptoms that began a year earlier and concomitant weight loss, following a traumatic event causing an overall impairment with that patient. Several medical evaluations/examinations looking for organic causes, were excluded. Interventions were implemented by a psychiatry, a psychotherapist using cognitive-behavior therapy (CBT), and a nutritionist, all in face-to-face modality, which were carried out weekly for the first three months, then biweekly and subsequently quarterly. each lasting approximately 40-60 minutes. After the set of pharmacological interventions and psychotherapy, a great improvement in the functionality of the patient was observed. Improvement was found with respect to eating in public, food variation and panic attacks. In the absence of guidelines, it is important to use standardized and replicable treatments in this population.

ABSTRACT

The COVID-19 pandemic and government measures have challenged family functioning (FF) and eating behaviors. In non-pandemic scenarios, FF has shown a protective role in diet quality. This study aimed to explore the role of FF in family members' food consumption during the first wave of the COVID-19 pandemic in Chile. We conducted a cross-sectional, non-probabilistic study. Chilean adults (n= 821) answered an online survey based on the Family Adaptability and Cohesion Evaluation Scales (FACES) IV and a scale of food intake perceptions (non-processed and processed food). The survey also included an open space for comments (n= 147). Most participants had a medium or high FF category. Participants with higher FF had a lower risk of decreasing the consumption of non-processed foods (RR= 0.58 for fruits and RR= 0.45 for legumes) and lower risk of increasing processed foods (RR= 0.37 for sugar-sweetened beverages, RR= 0.46 for snacks, and RR= 0.64 for chocolates) when compared to lower FF participants and controlling for per-capita income. Participants commented on changes in FF: some had more conflicts and others were happy to be together during the pandemic. Most participants indicated that they maintained or increased non-processed and processed food consumption compared to the pre-pandemic period. They perceived having the opportunity of eating healthier during the pandemic. However, the stress they suffered produced them to eat unhealthily. Therefore, FF had a protective role in diet quality during the pandemic. Policymakers should consider FF when promoting healthy diets, especially in health crises.

ABSTRACT

Different studies have related socioeconomic status to diet quality and the prevalence of nutrient deficiency risk. Among vegetarians, when an adequate diet is not followed, there is a possibility of having a low intake of certain nutrients, mainly vitamin B12, vitamin D, calcium, iron, zinc, omega-3 fatty acids and proteins. This study aims to evaluate the dietary intake of vegetarians according to their socioeconomic level in a region of Colombia. Participants completed a questionnaire in which they were asked about socioeconomic and anthropometric variables and the frequency of consumption of 48 foods and supplements. Subsequently, a subgroup of the sample was given two 24-hour reminders on different days of the week. We found that the lower the socioeconomic level, the lower the consumption of dairy products, whole grains, fruits and vegetables. Likewise, it was observed that depending on the type of vegetarianism, there are differences in the consumption of cobalamin, fiber and calcium. We conclude that, similar to omnivores, nutritional education is required to improve the choice of foods and to meet nutritional requirements, regardless of economic level.

ABSTRACT

Abstract Objective: To assess the association between abdominal obesity, food intake, sleep deprivation, and screen time in adolescents. Methods: This cross-sectional study was conducted with 432 adolescents aged 14-19 years, from public and private schools. Anthropometric and body composition measures included body weight, waist circumference, body mass index for age, waist-to-hip ratio, waist-to-height ratio, and body fat percentage. Abdominal obesity was defined by age and sex specific cut-off points for waist circumference. Food intake, screen time, sexual maturation, and sleep duration were evaluated by self-administered questionnaires. Results: The prevalence of excess body weight and abdominal obesity was 16.7% and 27.5%, respectively. Students in the adult phase (post-puberty), with inadequate waist-to-height ratio, high body fat percentage, and screen time ≥ 3 h/day were, respectively, 2.5 (95% CI 1.40-4.46), 7.44 (95% CI 1.08-51.46), 2.79 (95% CI 1.04-7.50), and 1.43 (95% CI 1.24-3.89) more likely to have abdominal obesity. Low intake of unprocessed or minimally processed foods was associated with inadequate sleep duration, while high intake of ultra-processed foods was associated with high screen time. Conclusions: Abdominal obesity was associated with the adult phase, short sleep duration and high screen time. The degree of food processing was associated with screen time and sleep duration. Assessing the risk factors of abdominal obesity may be a useful strategy for preventing cardiovascular disease in adolescents.

ABSTRACT

ABSTRACT OBJECTIVE To investigate the performance of food consumption markers of the Food and Nutrition Surveillance System (Sisvan) in assessing the overall dietary quality. METHODS The study was carried out based on the reproduction of responses to markers in 24-hour recall data from 46,164 individuals aged ≥ 10 years, from the 2017-2018 Household Budget Survey (POF). Seven Sisvan markers were evaluated, and two scores were calculated for each participant, based on the sum of the number of healthy food markers (beans, fruits, and vegetables, ranging from 0 to 3) and unhealthy (hamburgers/sausages, sweetened beverages, instant noodles/salt snacks/crackers, stuffed cookies/sweets/candies, ranging from 0 to 4) consumed. Linear regression analyses were used to assess the association between scores and diet quality indicators (ultra-processed foods, dietary diversity, and levels of saturated and trans fat, added sugar, sodium, potassium, and fiber in the diet). RESULTS The score of healthy eating markers increased significantly with increasing dietary diversity and potassium and fiber contents in the diet, while the opposite trend was observed for the densities of added sugar, sodium, saturated and trans fat (p < 0.001). The score of unhealthy eating markers increased significantly with the increase in the consumption of ultra-processed foods and densities of added sugar, saturated and trans fat levels in the diet, while an inverse trend was observed for potassium and fiber (p < 0.001). The joint analysis of the combination of the two marker scores showed that individuals with better performance (3 in the healthy food score, and 0 in the unhealthy food score) have a lower number of inadequacies in nutrient consumption. CONCLUSION Sisvan food consumption markers, quickly and easily applied and already incorporated into the Brazilian public health system, have good potential to reflect the overall dietary quality.

ABSTRACT

Abstract Objective: To evaluate the prevalence of ultra-processed food consumption and associated factors among children enrolled in the public school system of the city of Barbacena, Minas Gerais, Brazil. Methods: This is a cross-sectional study conducted with schoolchildren aged 7-9 years, of both sexes, enrolled in state public schools. Food intake was assessed using the Previous Day Food Questionnaire and the level of physical activity by the Previous Day Physical Activity Questionnaire. The listed foods were classified according to the extent and purpose of industrial processing, using the NOVA classification. Pearson's χ2 test, Fisher's exact test, χ2 with Yates correction, and Poisson regression were used in the statistical analysis, estimating the crude and adjusted prevalence ratio, with 95% confidence intervals. Results: The prevalence of daily ultra-processed food consumption was 69.6%. After adjusted analyses, the consumption of ultra-processed food was associated with the omission of breakfast, mid-afternoon snack, supper, low physical activity, and consumption of risk foods. On the other hand, consumption of in natura or minimally processed foods was associated with older age, the consumption of lunch, mid-afternoon snack, dinner, and protective foods. Conclusions: There is a high prevalence of ultra-processed foods consumption, associated with unhealthy dietary habits among schoolchildren. This highlights the need for nutritional counseling and educational actions, favoring healthy eating in childhood.

ABSTRACT

ABSTRACT OBJECTIVE To evaluate the evolution of household availability of regional foods in the state of Amazonas, their distribution according to sociodemographic characteristics, and potential differences when compared to the remaining areas of Brazil. METHODS Data on food acquisition for home consumption from the 2002-2003, 2008-2009, and 2017-2018 Pesquisa de Orçamentos Familiares (POFs - Consumer Expenditure Surveys) were analyzed, covering, respectively, 48,470, 55,970, and 57,920 households in Brazil, of which 1,075, 1,344, and 1,833 are in Amazonas. Foods were categorized into three groups: cassava and its derivatives, freshwater fish, and regional fruits. The study analyzed the amount of regional food purchased, expressed in relative household caloric share, for the entire area of Amazonas. Additionally, the data was stratified and analyzed according to sociodemographic variables, with differences assessed through the overlapping of 95% confidence intervals. FINDINGS The household caloric share of the total regional foods in Amazonas was 22.54% in 2002-2003, 18.18% in 2008-2009, and 6.49% in 2017-2018. Across Brazil, those percentages were much lower in the same period: 3.67%, 3.34%, and 1.82%, respectively. Changes in Amazonas were primarily attributed to the steep drop in the cassava and derivatives group, which decreased from 14.30% in 2002-2003 to 12.74% in 2008-2009 and further declined to 3.09% in 2017-2018. Additionally, there was a gradual decline in household availability of freshwater fish, decreasing from 7.30% in 2002-2003 to 4.85% in 2008-2009 and reaching 2.90% in 2017-2018. Households in rural areas and with lower per capita income presented a higher proportion of calories from total regional foods; this particular stratum also experienced the most significant reductions in their consumption. CONCLUSION During the study period, there was a significant decrease in the consumption of regional foods in Amazonas, particularly in lower income households in rural areas. Among them, the family reference person was typically a younger male with a lower educational background.

ABSTRACT

Current food consumption, characterized by increased intake of high-density foods and low nutritional value can negatively influence nutritional status and the emergence of health problems. This study aimed to evaluate the quality of the diet and associated factors in adult and elderly patients treated in a nutrition outpatient clinic. This is a cross-sectional study conducted from May to October 2021 at the Nutrition Outpatient Unit of the Fernando Figueira Institute of Integral Medicine in Recife, Pernambuco. The information of the sociodemographic and economic profile, lifestyle, clinical variables, and nutritional status was collected at the time of nutritional consultation (first consultation and/or follow-up consultation). The evaluation of food consumption was performed through the analysis of the Diet Quality Index - Revised (DQI-R). The sample consisted of 102 patients, and there was a higher frequency of overweight/obesity and very high cardiovascular risk. Regarding DQI-R, 72.6% of the sample has a healthy diet. The follow-up time was positively correlated with the quality of the diet (ρ 0.199; p <0.045); and regarding fasting blood glucose, there was a negative correlation with the consumption of total cereals (ρ -0.229; p <0.050) and a positive correlation with the consumption of milk and derivatives (ρ 0.265; p <0.023). Regarding anthropometric variables, negative correlations with the consumption of whole grains and cereals was demonstrated. Food consumption is considered complex and requires the evaluation of factors that may interfere with it. Nutritional monitoring, modifying, and improving the quality of food, favors the adoption of a healthier lifestyle, which relates to the prevention and/or control of chronic non-communicable diseases and their complications.

ABSTRACT

ABSTRACT Objective: To prospectively analyze changes in the frequency of individual food consumption of adults/elderly people according to different food insecurity outcomes over time. Methods: Population-based longitudinal study carried out in 2011 (358 individuals) and 2014 (301 individuals) in a municipality in the northeastern semi-arid region. The frequency of food consumption of 37 foods in adults/elderly was assessed using the Food Frequency Questionnaire and food insecurity using the Brazilian Food Insecurity Scale. Differences in the proportion of frequency of consumption of each food at baseline (2011) and follow-up (2014) were calculated according to longitudinal categories of change in food insecurity. The McNemar test for paired samples was applied to estimate differences between 2011 and 2014. Results: Among the individuals studied, 38.9% and 30.6% were classified in food security and food insecurity in the two periods (2011 and 2014), respectively, and 23.2% changed from food insecurity in 2011 to food security in 2014. Increase in the frequency of food consumed in the three groups of food insecurity outcomes. Only in the food insecurity group at both times, an increase in the consumption frequency of soft drinks and industrialized juices was observed. In the three groups, when comparing 2011 and 2014, there was an increase in the consumption of local agricultural foods, such as mangoes, sweet potatoes and a decrease in the consumption of pumpkin. Conclusion: Overcoming food insecurity results in positive changes in food consumption, and seasonality is a factor that promotes and limits food consumption.

ABSTRACT

A 26-year-old female was hospitalized with complaints of repeated vomiting and weight loss (18 kg in 8 months); and incessant belching and nausea/regurgitation for 30 days subsequent to her COVID-19 treatment. After all the normal medical radiological reports, appropriate psychological and psychiatric evaluations were done. Incessant belching warranted a simple and flexible cognitive psychotherapy for histrionic personality with a therapeutic assessment technique. A total of 6 h of psychotherapy was delivered in three sessions. The belching and vomiting came down by 75% after the first extensive session, and almost 100% after the second session. However, mild belching reappeared, and after the termination-cum-booster session belching subsided till discharge and the first follow-up after 2 weeks

ABSTRACT

Purpose: Obesity has become a growing public health issue worldwide. Studies have shown that eating rate is one of the most important factors to consider in the strategies to prevent and/or treat obesity. Eating rate can be reduced through different strategies, such as an increase in oro-sensory exposure, the modification of food texture, and an increase in the number of chewing cycles. The aim of this systematic review was to analyze the available evidence regarding the effect of chewing behavior modification on the parameters that contribute to obesity. Methods: A systematic search was done on the electronic databases Pubmed, Cochrane Central Register of Controlled Trials, and Scopus, using the terms "mastication", "chewing", "chewing speed", "prolonged chewing", "number of chews", "masticatory cycles" "satiety" "satiety response" "appetite", "appetite regulation", "nutritional status" and "obesity". Results: A total of 23 intervention studies were selected that intervened in the participants' chewing behavior, either by reducing the eating rate, increasing oro-sensory exposure, food hardness or the number of chewing cycles. In most studies these interventions were effective at reducing food intake, subjective appetite and improving the plasma levels of satiety-related hormones and metabolites; moreover, they reduced body mass index in the long term. Conclusion: The currently available evidence seems to indicate that modifications to chewing behavior can bring with it a myriad of benefits for the treatment of obesity.

ABSTRACT

Lockdown and social distancing due to COVID-19 affected the mental health and lifestyle of the population. However, there is insufficient evidence of alterations in eating behavior. Our study seeks to describe the relationship between eating behavior and eating habits among Chilean adults during the confinement period. A sample of 760 Chilean subjects was analyzed, who answered surveys using Google Forms, considering demographic characteristics, social distancing, dietary habits and EB. More than half of the participants consider that their dietary intake increased during confinement. Changes in dietary intake were analyzed according to food group, and a decrease in the consumption of fish, fruits and dairy products was observed, while legumes, processed foods and soft drinks showed an increase, which represents risk factors for the development of cardiovascular diseases. When analyzing eating behavior, a greater difficulty in stopping eating was observed when faced with external stimuli; increased intake associated with complex emotional situations, and when isolating the group that decreased their intake of unhealthy foods, a greater ability to limit their intake for weight control was reported. Our results are similar to other studies, and they reinforce that confinement is related to eating behavior, leading to changes in eating habits, which indicates that, at the public health level, post-pandemic nutritional strategies, should be focused on regulating eating behavior in order to guide habits towards healthy eating.

ABSTRACT

Malnutrition is a frequent clinical and nutritional problem in hospitalized patients, and early detection of nutritional risk is essential, as it allows the multidisciplinary team to immediately start an adequate dietary approach, minimizing the synergism between malnutrition and other clinical factors. Objective: to determine the nutritional risk, associating it with the nutritional status, lifestyle and clinical variables of hospitalization of patients hospitalized by the SUS, comparing such conditions between the sexes. Methodology: this are a cross-sectional study. To assess the nutritional risk of hospitalized patients, the Nutritional Risk Screening (NRS) 2002 protocol was used. In addition, anthropometric, clinical, sociodemographic and dietary data were collected for comparison with nutritional risk. Results: the sample consisted of 100 patients, 50% adults and 50% elderly, with a mean age of 56.11±19 years, the majority being male (51%). Nutritional risk was present in 34% of the patients evaluated, being 58.8% female and 41.2% male. There was no difference in the other variables when compared between the sexes (p>0.05). The highest prevalence of risk observed was in the elderly (27%). Conclusion: nutritional risk was more prevalent in elderly patients, but there was no difference between the sexes. It is known that the presence of comorbidities and an inadequate acceptance of the diet by them can lead to a significant worsening of the nutritional status.

ABSTRACT

Abstract: Background: Low health-related quality of life has been observed in patients with eating disorders, but little is known about how treatments, such as cognitive behavioral therapy, impact the perceptions and experiences of patients. Objective: To explore the health-related quality of life in youth diagnosed with eating disorders receiving cognitive behavioral therapy. Materials and Methods: Qualitative study with multiple case methodology and intentional sampling of five youth receiving cognitive behavioral therapy for eating disorders in two treatment programs in the city of Medellín. These youth were given an in-depth interview related to health-related quality of life. Results: Although cognitive behavioral therapy created restrictions in the lives of the participants at the beginning of the treatment, improvements were observed in different areas of health-related quality of life, as treatment progressed: the ability to do things, move or take care of oneself; cognitive and academic skills; emotional state; social relationships; self-perception and health status perception, including mental health. Conclusion: The participants progressively improved their health-related quality of life as a result of cognitive behavioral therapy, both by reducing the problems associated with eating disorders and by increasing life skills.

ABSTRACT

The evaluation of the food consumption of the Brazilian population is a long-standing concern. The first comprehensive data on the diet of Brazilians were those from the National Study of Family Expenditure (ENDEF) between 1974-1975. The survey captures the data of one week family consumption, using weighing method. Since then, data from the Family Budget Survey (POF) have been used to assess diet in Brazil. The first POF was carried out in1987-1988, followed by 1995-1996 and 2002-2003. Estimation of consumption is based on household purchases in a period of one week. Thus, meals eaten away from home are not considered, and therefore, do not allow the assessment of individual consumption.. In order to solve this limitation, the last two POF carried out in 2008-2009 and 2017-2018included a new block of data that collected information from individual food consumption in a subsample of the survey. These data allow making individual-level estimates, characterizing food consumption by sex, age group, and other sociodemographic variables. Development and analytical strategies of the 2017-2018 National Dietary Survey (NDS) included in the Family Budget Survey is shown in this thesis. Thus, the objective of this work is to document the process of selection and adaptation of the instruments, the food database for energy and nutrient analysis and the necessary harmonization with previous NDS of 2008-2009. Also, the foods most frequently consumed in the two surveys and the evolution of the consumption in the last decade was analyzed. For NDS 2017-2018, two 24-hour recalls were collected on non-consecutive days of 46,164 with 10 years of age or older, representing a subsample of 34.7% of the general POF. All methodological changes from one survey to the other, such as: instrument to collect data, food composition databases, new variables added are presented. In addition, results of change in the foods most frequently consumed by the population is presented. Changes in data collection improved the instrument that had the lowest underreporting in the most recent survey. The change in the composition table for Brazilian Food Composition Table (TBCA), in reanalysis of the 2008-2009 data, showed changes in energy, macronutrient and mineral estimates of less than 15%, with the exception of trans fats and selenium, with average changes of 40% and 52% lower in the TBCA. Comparing the two surveys, it was found that although the results show that the Brazilian population continues to eat rice, beans, coffee, bread, beef and vegetables as the six main food items, there was a reduction in the frequency of consumption of rice, beans, beef, breads, fruits, dairy products, processed meats and soft drinks and an increase in the consumption of sandwiches, regardless of gender, age and income. The two biggest changes were related to the reduction of beans and the increase of sandwiches, which was estimated to be more than 4 times more likely to increase in elderly Brazilians. A positive trend was the reduction of soft drinks in all income classes. The changes observed in the diet of Brazilians in this 10-year period are consistent with the policy of reducing sweetened beverages and with the reduction of family income in the period. The results indicate a worsening in the Brazilian diet.

ABSTRACT

ABSTRACT Objective To assess and identify factors linked to the accuracy of patients' food intake estimations through a self-monitoring instrument filled in by the patient. Methods This cross-sectional study approached adult hospital patients subjected to regular or therapeutic diets. The actual food intake percentage was obtained by the ratio between the actual food intake and the amount of food served x 100. Patients had to complete a food chart including 9 circles representing plates of food in percentages (increasing scale of 12.5%) to represent their food intake at lunch and/or dinner. The Bland-Altman method assessed the agreement between the actual and the estimated values. The associations between variables (age, sex, hospitalization day, diet prescription, amount of food served and actual food intake percentage) and the accuracy of the food intake estimation (adequate ±10%, overestimated and underestimated) were evaluated through univariate multinomial logistic regression. Results Ninety-six patients were evaluated (51.0% male; 44.0±15.8 years of age). The Bland-Altman analysis showed good agreement between the actual and the estimated food intake. The actual food intake percentage was the only variable associated with the accuracy of the food intake estimation. Conclusion Most patients (~70%) adequately estimated their food intake using the 9-point food chart tested. Furthermore, the only factor linked to the accuracy of the food intake estimation was the actual food-intake percentage. These findings provide preliminary support for the usefulness of this instrument. However, it must be tested in a representative sample of hospitalized patients.

ABSTRACT

ABSTRACT Objective To present changes in the estimated amount of food intake in Brazil between the 2008-2009 and 2017-2018 National Dietary Surveys. Methods Food intake data from the 2008-2009 and 2017-2018 surveys were used to highlight the differences in the frequencies of foods mentioned, the number of the measurement units mentioned, and the frequency of measurements that were incompatible with the reported food and were exchanged by the most mentioned measurement (standard measurement), as well as to describe the updates performed in the database between edits. Results The elaboration of the 2017-2018 referenced measurement table was based on the 2008-2009 table, which was revised and updated. In the 2008-2009 survey, 9980 household measurements were mentioned for 1970 types of food and preparations, while in 2017-2018 there were 11050 and 2534, respectively. While in 2008-2009, 2.8% of citations were replaced by the standard measurement, in 2017-2018, only 0.7% of food items needed to be replaced. Conclusion The procedures used to estimate the amount of food intake between the surveys allowed updating the table of household measurements and minimizing errors in the estimate of this amount, with a reduction in measurement units that were inconsistent or incompatible with the aforementioned foods.
